Place a sheet of graph paper vertically onto a table.
Write the child's name along the top of the graph paper using colored pencils. Draw a line under his name.
Draw a rectangular box along the side of the sheet, attaching together around six to eight of the graph squares to make a long section. Write a positive behavior inside the box, such as "Used Polite Word." Repeat this process until you have all of the boxes and behaviors listed that you require.
Section off 14 boxes underneath the child's name to create a long rectangle. Separate the rectangle into groups of two.
Write the days of the week inside the grouped boxes, one day for each set of two squares.
Place a sticker into the boxes on the graph chart for each day that your child demonstrates the good behavior. Repeat with each behavior listed on the chart.
