Use a notebook or spreadsheet to keep track of your student's activities. Using a notebook is a lot like keeping a journal, where all of the information for one day is above the information for the next day. Having a spreadsheet means that each day's information is side by side in individual columns.
Place the date on top of each entry. You will need to know exactly which day your student participated in her activities, in case you ever need to provide detailed information to the state or to a college. It will also help you easily organize your student's activities into weeks, months and years.
Note the time and duration of each activity. This will allow you to know when and how long your student participated in an activity. This is especially important in situations where a total number of hours are required. For example, if your state requires a certain number of hours for P.E. or if a prospective college wants to know how many hours your student engaged in volunteer work.
Record activities each day for each child you are homeschooling. Filling in your student's activity log should be a daily activity for you. If you have a child that did not participate in an activity for the day, you can note that as well. This way you have a clear record of when each of your students did or did not participate in an activity, without confusion or room for error.
