How to Head an Autobiographical Statement Paper

An autobiographical statement is used as part of many universities' graduate school application process. In it you are expected to tell the school's admissions department about yourself, your goals and your reasons for attending graduate school. These essays should not be biographical and instead focus on your accomplishments and what makes you stand out from the rest of the applicants. This paper can also be called a statement of interest or a personal statement, and the header for this document should be simple and not distracting from the content.

      Read admissions requirements from the university to be sure there are no requirements for the header of the autobiographical statement. Many schools require statements to be submitted in a text format online but do not ask for specific header information. If there are requirements, follow those rules exactly.

      Write your first name, middle initial and last name on the first line of your paper. This should be aligned to the left margin of the page.

      Write "Personal Statement" or "Autobiographical Statement" on the second line. Select the same title for the paper that the school uses in its application information.

      Write the university name and the specific school you are applying to. For example, write "Michigan State University College of Engineering."

      Write "Degree Objective" followed by a colon and the degree you hope to obtain. For example, an applicant might write "M. Eng.," which stands for Masters of Engineering. Abbreviations are acceptable for this portion.

      Press the "Enter" key once to put an extra line between your header and your essay, and double-space the header. Write your statement in below the header.
