Next if the question is asking about what to teach the patient to monitor for bleeding or how you would assess for bleeding, then you need to teach the patient to monitor for or monitor the patient for signs of internal bleeding by checking for bloody urine, tarry colored stool and coffee ground emesis.
Finally if the question is heading toward a medication precaution scenario, then you would want to find out if the patient has been taking aspirin or aspirin containing products that increase the risk for bleeding. The patient should avoid these.
