Questionnaires are a useful survey assessment tool. Questionnaires include a list of questions for people to fill out by hand or on a computer. They ask a variety of questions: yes/no, multiple choice, order of preference. The specific types of questions vary according to what information you need. Questionnaires are usually sent out to many people, not just one or two.
An interview is a type of survey assessment. Interviews are structured conversations between one or more interviewer and one or more interviewees. The interviewer asks the interviewee for her view on some specific subject or set of subjects. The interviewer transcribes the interview later that day and records the information.
Observation is a third type of survey assessment. Used often by teachers or company managers, observation involves someone watching the proceedings of a class or a workday. The observer records relevant information about what is happening. The observer may stop the observed person to ask questions or ask for clarification.
